How do I inform the Office of Bar Admissions if I have a name change or correction?

You are offered the opportunity to change your name or make any corrections to your name in the letter that is initially sent to you by the Office of Bar Admissions confirming receipt of your bar application.

You may also make a name change by mailing or faxing a name change form to the Kentucky Office of Bar Admissions.  Name changes by e-mail to info@kyoba.org will also be accepted.

NOTE: Name changes must be made no later than 30 days prior to the release of bar exam results.  Any change made after the release of the bar results can only be made upon a formal motion for a name change to the Supreme Court.
