What forms are required to be submitted at the time I file my bar application?


 Along with the completed application, you must submit the following:

1.    Completed Application.

2.     Application Fee (See Fee Schedule):  Appropriate fee in the form of a certified check, cashier’s check or money
        order
made payable to the Clerk of the Supreme Court of Kentucky MUST accompany the application. 

             3.    Authorization & Release Form:  There must be two signed and notarized Authorization & Release forms
                    accompanying your application.

             4.    Credit Report:  On 12/14/05, the Character & Fitness Committee of the Kentucky Office of Bar Admissions adopted
                    a new policy wherein applicants applying for admission to the Kentucky Bar must obtain a copy of their own credit
                    report to be submitted with their applications for admission.  **See application for further information in obtaining
                    a credit report.

                   This policy is in effect for all applicants applying for admission by examination and any applications received for admission
                   without examination (SCR 2.110), limited practice (SCR 2.112), limited certificate of admission (SCR 2.111) and
                   reinstatement applications (SCR 3.500 & 3.510).

             5.   Computer Testing Registration Form and Fee. (See Computer Testing Info)

 6.    Non-Standard Testing Accommodations Application (if applicable).
