What forms are required to be submitted at the time I file my application?


 Along with the completed application, you must submit the following:

1.   Application Fee:  Application fee of $1,200 in the form of a certified check, cashier’s check or money order made payable made payable to the Clerk of the Supreme Court of Kentucky.  There will be no refund of fees in case of denial or withdrawal of an Application

2.   Authorization & Release Form:  There must be a signed and notarized Authorization & Release form.
      accompanying your application and if applicable, the Authorization to Release Medical Records Form.

3.   Credit Report:  On 12/14/05, the Character & Fitness Committee of the Kentucky Office of Bar Admissions adopted a new policy wherein applicants applying for admission to the Kentucky Bar must obtain a copy of their own credit report to be submitted with their applications for admission.  **See application for further information in obtaining a credit report.

                  This policy is in effect for all applicants applying for admission by examination and any applications received for admission
                  without examination (SCR 2.110), limited practice (SCR 2.112), limited certificate of admission (SCR 2.111) and
                  reinstatement applications (SCR 3.500 & 3.510).
