Where do I find contact information for requesting criminal history records, driving records and certificates of good standing

Lists of addresses are provided in the General Instructions of the Application for Admission Without Examination to assist you in obtaining your state criminal history records, driving history records and certificates of good standing.  

NOTE:  To obtain a disciplinary from the state(s) in which you are admitted, you will need to contact the appropriate for that jurisdiction(s).
